School Wellness
In addition to health services, School District 69 is committed to promoting the safety and wellness of all students. In an effort to maintain the goals set forth in District 69’s School Wellness Policy (6:50), food within our learning environments should be healthy. The Safe and Healthy Schools document includes information for parents regarding snacks and school celebrations.
Health Conditions
If your child has life-threatening anaphylactic allergies (including food allergies), asthma, diabetes, seizures or other health conditions, please contact your school office. These conditions may require health care or action plans to be completed by you and your child’s physician. Although your medical provider may use different forms, the forms below are also acceptable.
In addition to your child’s action plan, a School Medication Authorization Form is required to administer your child’s medication at school. The forms below must be submitted to the school’s office.

Medications at School
If it becomes necessary for your child to take medication at school (prescription or non-prescription), all medications must:

Be brought to the school by an adult along with the School Medication Authorization Form completed by you and your child’s doctor. This form must be completed prior to medication being administered at school. Medications may not be sent to school with students.
