Faith's Law Employment History

Illinois law (105 ILCS 5/22-94) requires that school districts conduct a sexual misconduct background check on new hires. The District uses a third party, Verifent, to conduct this background check.

Employees are required to provide the names, contact information, and other relevant information related to their current/former employer(s).

Please follow the steps below to start your request for Faith’s Law Employment History Review:

  • Visit Verifent website
  • Click “Register” under the “Employee” section on the right side

Step 1: Choose Your Hiring Entity

  • On the left side of the page, under “Actions”, choose "Request EV"
  • Click "Choose Your Hiring Entity"
  • Enter "Skokie School District 69 - IL"

Step 2: Experience Verification

  • On this page, choose the forms you need your Former Employer(s) to complete and provide your information
    • Faith's Law Employment History Review & Self-Disclosure Form (required for all)
    • Certified Experience (teacher hires with prior teaching experience)

Step 3: Former Employer(s)

  • Click "Enter ALL Former Employers" (per step 2 above)
  • Enter all former employer(s). If you are unable to find one of your former employers, click “Enter New Former Employer.”

Confirmation

  • Verifent will notify you of the progress of your requests through email.
